Pfizer Vaccination at Liverpool Boys High School

PFIZER PROGRAM

NSW Health will be at Liverpool Boys High School for any students who want the Covid-19 Pfizer Vaccination.

Dose 1: Monday 22 November 2021

Dose 2: Monday 13 December 2021

Students will be supervised by NSW Health and Liverpool Boys High School staff in the Hub at School.

Consent form in English 

Speak to/text message/email Ms Stanton if:

  • You have already had Dose 1 but would like Dose 2 at school
  • You need more information
  • You need the consent form
  • You need the consent form in another language
